| SENDING STUDENTS HOME |
- Students are to go to the Health Office if they feel ill or are injured.
- If it is necessary to send the student home, the office staff will call to make the arrangements.

| MEDICATION |
If it is necessary for your student to take medication during school hours, obtain the proper forms from the health clerk or download them from the bottom of this page.
- The forms must be completed by your physician.
- The medicine must remain in the prescription bottle and will be administered according to the doctor's directions by the health clerk.
- Students are NOT to have any medication (including aspirin) in their possession.

| Please notify the school should emergency information change during the school year (name, address, home/work phone number, etc.) It is vital that your child's emergency records be up-to-date in case an emergency arises and parent contact must be made. |
